Troubleshooting • Pi 5 boot issues - needs 10+ power cycles to start, only red LED

Hardware:

Raspberry Pi 5
Pi 5 power supply
USB stick with Homebridge OS flashed via Raspberry Pi Imager

Problem:
My Pi 5 won't boot reliably. I have to plug/unplug the power cable about 10 times before the green LED finally lights up and the system boots. Most of the time I only get the red LED (power), but no green LED activity.
What I've checked:

Power supply: Using official Pi 5 PSU
Connections: All cables properly connected
Boot files: All firmware files (bootcode.bin, start*.elf, fixup*.dat) are present on boot partition

Current config.txt:

Questions:

Is this a known Pi 5 bootloader issue?
Could it be related to the Homebridge OS image compatibility with Pi 5?
Should I update the firmware files on the boot partition?
Any specific Pi 5 config.txt settings that might help with boot stability?
